Optical

50% up to $
250 per year

Glasses, Lenses and Exams

50% reimbursement of actual costs incurred up to $250 per person per policy year or for all Insured Children collectively.

No-claiming period: 6 months

Medical (including GP visits)

50% up to $
750 per year

GP visits, Specialists, and Diagnostic Tests and health screening (excluding mole mapping)

50% reimbursement of actual costs incurred up to $750 per person per policy year or for all Insured Children collectively.

No-claiming period: 3 months

Complementary Medical

50% up to $
400 per year

Includes physiotherapy, chiropractic, osteopathy, podiatry, chiropodiatry, acupuncture

50% reimbursement of actual costs incurred up to $400 per person per policy year or for all Insured Children collectively.

No-claiming period: 3 months

Mental Health

50% up to $
1000 per year

Psychologist, psychotherapist, psychiatrist, counsellor

50% reimbursement of actual costs incurred up to $1000 per person per policy year or for all Insured Children collectively.

No-claiming period: Not applicable

Hospital Treatment

50% up to $
700 per year

50% reimbursement of actual costs incurred up to $700 per person per policy year or for all Insured Children collectively. Includes reimbursement of excess on eligible hospital cover policies ($500 maximum applies per claiming year**).

No-claiming period: 3 months 

 

Major Diagnostic Investigations

50% up to $
600 per year

Includes Angiogram, CT scan (including myelogram), MRI scan, PET scan

50% reimbursement of actual costs incurred up to $600 per person per policy year or for all Insured Children collectively.

Medical Appliance

50% up to $
400 per year

Specified items prescribed by a GP, specialist or hospital.

50% reimbursement of actual costs incurred up to $400 per person per policy year or for all Insured Children collectively.

No-claiming period: 3 months

 

Orthodontic Treatment

30% up to $
750 for each Insured Child

30% reimbursement of actual costs incurred up to $750 for each Insured Child, up to a policy maximum of $1,500 for the duration of the Member’s Membership.

No-claiming period: 12 months

Sick Leave Without Pay Grant

50 per week

$50 per week plus $5 for each Insured Child up to a maximum of $60 per week for 26 weeks for the Member.

No-claiming period: 12 months

Birth Grant

$200 for each live child born to a Member or their partner/ $200 for each child adopted by a Member or their partner

Bereavement Grant

$1,000 on the death of an Insured Adult or Child (including still birth).

Bowel Screening Kit

One kit every three years

Children do not qualify for this benefit.

Many of these services are available at no additional cost to members with an eligible UniMed Health Plan.

Simple, non-urgent health questions answered by a New Zealand GP, usually within 1 business day. 

Skin Checks

Monitor your skin and detect skin cancer early using the SkinVision mobile app.

Nutrition Coaching

One-on-one virtual sessions with a registered dietitian to help you build energy, manage weight, and stay on track.

Fitness Coaching

One-on-one virtual sessions with fitness experts to understand your specific situation and your fitness goals.

Personalised coaching programs from Osara Health for those living with cancer or those who are caregivers.

(*) This is the sum of selected benefit maximums included in the Primary Care plan and represents the maximum amount per year for each Insured Adult. The sum includes benefit maximums for Optical, GP, Specialists and Tests, Complementary Medical, Mental Health, Major Diagnostic Imaging, Hospital Treatment, and Medical Appliance. It does not include benefit maximums for hospital excess, orthodontic treatment, bereavement grant, birth grant, sick leave without pay grant, or bowel screening kit. Children’s benefits apply collectively to the children covered under the plan. 

 (**) The Primary Care plan can also be linked with an Approved Hospital Cover policy to take advantage of the excess reimbursement for hospital treatment ($500 maximum limit applies).
 
Primary Care is underwritten and administered by Union Medical Benefits Society Limited (UniMed). Any cover issued in response to this application is subject to the relevant Health Plan document and UniMed’s Terms and Conditions.
